FORMS:- I need a copy of my (child's) baptism certificate - email [email protected] with the person's name, birth date, parent's name, and how you would like to receive the certificate (pick up, mailed to you, or mailed to another parish).
- I need a year-end tax statement - Email [email protected] and provide us with a good mailing address. [End-of-the-year statements are sent out by January 31st. Email if you have not received it by March 1st.]

SACRAMENTS:
- I need information about baptizing my child - CLICK HERE to learn the requirements for baptism.
- I need to sign up for a baptism preparation class - classes are offered every 3rd Thursday of the month, except December. Call the parish office (817-478-8206) to sign up for the class.
- I/my child am over the age of 7 and have not been baptized - you/your child will want to sign up for RCIA /RCIA adapted for children to prepare for the sacraments. - I need to schedule a funeral / memorial Mass - scheduling and planning funerals are done by Deacon Kevin Bagley. Contact Deacon Kevin at 817-478-8206 x 211. Funerals are typically scheduled Monday - Saturday between 10 AM - 2 PM.
